If he cleaned it first, you should have used it.
If he didn't clean it, and wouldn't let you clean it, then you were justified in not using it.
But in all aspects you should (and he should) have been courteous.
As in: "thank you for the loan of the mouthpiece. I'm really sorry I forgot mine. May I please go to the restroom and wash it off first? I'll wash it again before I give it back to you."
