I just got that same letter today and mine mentioned but didn't include an application form either! I'll call tomorrow to see if the office can fax the form. If successful I'll post it on the web for eveyone. Otherwise I'm thinking of just sending in a generic application form with my check. (Which better go out soon to make it by the 20th.)
