Don Stauffer includes both a picture of himself playing one and a catalog page for a 34J Symphony Grand Bass from the thirties. It had four front-action valves and an upright bell, and was available in CC or BBb. These photos are in "A Treatise on the Tuba" self-published by Donald Stauffer, and (surprisingly) available from Amazon (at least when I bought mine).
