Regarding mouthpieces,
Some small ones are the Bach 32E, Conn Helleberg 7B, and the Yamaha John Griffiths model.
The Yamaha-Griffiths is really TINY...I mean tiny. Although not originally intended to be used as a beginner mouthpiece, I believe that it would work well for small tuba students.
