In the case of the Warburton-Neilan that Terry made a little while back, it was a lot more expensive than a stock mouhtpiece for a couple reasons:

He had to specially make the oversized blanks, it is not something he would normally have sitting around.

Because he only made a few and they required a special setup of the machine, he could have been cranking lots of trumpet mouthpieces in the time it took to get his lathe configured for the huge mouthpieces, cut a few, and then set his equipment back to normal again.

I am sure that if he started making them a hundred at a time, the price would have been dramatically less.

He may still have some blanks left and be able to make one if anyone out there is interested.
