What Tony has said here is very important. I think too many people neglect the extreme low register playing after working your high register. One way to work the high register is to use the Melodious Etudes he mentioned and take it one phrase at a time, 1st down an octave, next as written, then down two octaves. Keep repeating this throughout an etude and you will work each of these registers equally, without hurting yourself in the process.
