Try using the tuner more sparingly and focus more on playing intune with yourself as a start. Play 1 octave scales, starting and ending on the tonic pitch with a sustained sound. While holding that note at the top and bottom of the scale, imagine singing the rest of the scale going back to the other octave, then play the rest of the scale. By focusing on hearing the rest of the scale in relation to the tonic pitch, hopefully you will play the notes with better relative intonation.

I have tried to focus on indivudual notes in the past with a tuner and always had very poor results. Not until the notes are put in context did they start to lock in better. Also try the same thing with arpeggios. Hold the bottom and top notes while mentally singing the rest of the arpeggio, then actually play it.

If someone else, who you believe to be much more proficient than you, can play your horn with decent intonation and the horn is not too beat up, then it might be an issue of mental training.
