Recently flew cross country on Delta - no surcharge, no questions asked for Walt Johnson case with tuba. Flew last year pre-9/11 with no problems on USAIR, Delta, and in Europe on SAS.
I think the "discount" airlines - Southwest or JetBlue, for instance - may be inclined to charge extra.
Also think one's status in frequent flyer clubs - Million Milers, etc - might result in better "wink and a nod" treatment on this matter. (It doesn't apply to me, but at least I think this might be true).
