Try a King 26. There is no better mouthpiece for a King sousa than this. I am here in Ohio also if you want to try one out. A Mike Finn MF1 is very similar to the King 26 also. It is a little heavier than one but it is a great mouthpiece also. I have no advice on reducing the pain of carrying a sousa. I am currently working with a phyiscal therapist and orthpedist to ease the joys of sciatica of too many years of carrying one myself.
