A little while ago I needed a replacement button for a 19th century "Melchior De Vries" valve trombone. Strangely the local music shop was out of stock of matching parts, but a local repairman built one from scratch overnight, and didn't even charge for it. I also had a pair of matching valve caps for a King 8B bass trombone made in a specialist machine shop, again for free.

I guess what I'm trying to say is that according to the people I have asked to do it, it's a small job, especially if they use a fairly standerd thread (that valve trombone was very much not standard), and any repaitman should be able to make something that you won't be able to tell from an original, as long as you don't want a pearl inlay.
