The only 5-valve all-piston CC I have ever seen was an old King. It was not a "cut" job but a factory original, perhaps a special order. I can't remember exactly how the fifth valve was placed. It may have been arranged so that the left hand could operate it on the side, as on the old top-action Besson compensators. This King CC was a side action, however. I saw it at Pat Landolfi's house in the mid '80s.
