When you breathe in, do you breathe out without hesitation? You should breathe in until the point you play the note. Sometimes if there is a slight preparation for the note/hesitation of breath, you can have these problems. The breath in and the breath out should be one movement. The change of direction of the airstream should be immediate. Add that to the breath attacks, and see what happens.
