The really old Doug Elliot Catalog I have indicates that Doug was making Cimbasso mouthpieces w/ a shank & in-between cups similar to what you describe.
I can also attest from my dealings w/ him that if a shank don't fit, he'll make a custom one for no additional charge (following the guidelines of his generous return policy, of course) He did that for me and it fit perfectly. all this was accomplished over the phone!
