I've done it a couple of different ways. If you have the tradtional S-linkage, you'll have a slot in the stop arm. As Joe suggests, it's easy to slip the nut into the slot, with the bolt through the ball, the cone, and the top half of the stop-arm clevis.

On the paddle lever, just unscrew the old shaft, drill out the hole, and put the bolt through the hole. Pro repair guys have taps to rethread holes, but I just use the bolts and have never heard any complaints.

If you have stop arms designed for Presta or other ball-joint system that you are replacing, you might have a conflict with the larger Du-Bro joints. I threaded 4-40 all-thread into the stop arm on my Miraphone, put a half inch of brass tubing over it, and the ball above that, with a nut above that. I then soldered the tube to the stop arm to make it more rigid. That rig has lasted ten years reliably.
