I've used a small handscrew clamp (like a Jorgensen available at a hardware store) with wooden jaws to grip the cap. Take it easy with the clamp pressure; you don't want to distort the cap and casing, just grip it firmly. If the clamp slips, no biggie; the wood won't scratch a thing.

Two things you can do to for future situations. Put some Permatex anti-seize compound (available at an auto parts store) on the (cleaned) casing threads. And keep a rubber jar lid remover (sometimes given away as advertising gimmicks) tucked away in your gig bag to help you get a little traction the next time you have to get the cap off.
