Personally, I wouldn't even consider it unless my oral sugeon was intimately familiar with low brass playing, and could assure me that it would not ruin my playing. I would also want him to have done the surgery before on a low brass player, and then I would talk to that person to see how the surgery affected their performance.
TMJ had been quite bothersome for me for about 15 years. I would get intense pain in the jaw (both sides), and stress headaches in the back of my head and neck. Surgery had been suggested a few times, but I refused. On the suggestion of a friend, I went to see a chiropractor and in two months I was virtually pain free. I would suggest that you try a chiropractor before you choose to have surgery -- but make sure the chiropractor you choose is familiar with TMJ
