If I had to play that piece I would have four choices:
(1) Go out and spend roughly 10 kilobucks on a five-valve tuba in BBb or CC.
(2) Spend about 1 kB having a rotary 5th valve and crook built that I could slip into the 4th valve tubing in place of one of the tuning slides, just for special occasions such as this one.
(3) Scrounge some tubing and make an extra-long 4th valve tuning slide that would temporarily make it into a "quint" valve, approximately. Use conventional 3-valve fingerings for all the notes down to FF, finger the DD 2&4, the DDb 12&4, the BBB 1234, and the CCC 234.
(4) Spend no time or money on alterations, and use the "false pedal" note with 2&3, hoping that nobody notices the less-than-perfect sound quality.
