At a Patrick Sheridan master class I attended last fall, he suggested to take your mouthpiece and run the gamut of your range on it. He said that any notes that skip or jump or chip or whatever when you do this on your mouthpiece are the notes you will skip or junp or chip or whatever when you play them on your horn. If this is the case, it's you and not your horn, as others in this post have suggested, and changing horns will only be a temporary fix. This exercise has helped me a lot. IMHO.
